急性大咯血

2005年1月-2007年10月于德班市Inkosi Albert Luthuli中心医院胸心外科就诊的适合行急症肺部切除术的大咯血患者被纳入研究组。根据急症临床评估，判断急诊肺部切除术前是否行BAE.如果患者行BAE，依据大咯血复发时间行肺部切除术。适合行急症肺部切除术的大咯血患者共61例。41例未行BAE直接行急症肺部切除术，1例术后复发轻度咯血。余并发症包括2例死亡，1例术后脓胸，1例胸部切口重度感染。20例术前行BAE，15例患者（75%）行BAE后复发咯血。术后无复发咯血。余并发症包括1例死亡，2例术后脓胸。

该回顾性研究显示：针对影像学资料显示病灶局限的大咯血患者可直接行急诊肺部切除术。数据显示针对不适合行急症肺部切除术的患者，BAE是一种有效的缓解性治疗措施。
