心力衰竭(heart failure,HF)的全球发病率超过6430万例,其中约50%的患者为射血分数保留型心力衰竭(heart failure have preserved ejection fraction,HFpEF)。HFpEF是一种复杂且日益普遍的临床综合征。在影响HFpEF临床结局的诸多因素中,营养状态已成为关键决定因素。营养不良常见于HF患者,并与不良临床结局相关。值得注意的是,肥胖虽被视为心血管疾病的危险因素,却在慢性心力衰竭患者中表现出生存获益的"肥胖悖论"现象。然而,这些营养极端状态影响HFpEF患者的具体机制(特别是性别特异性结局方面)尚未明确。营养风险筛查2002(NRS-2002)是欧洲临床营养与代谢学会推荐的标准化筛查工具。既往研究已证实该评估在慢性心力衰竭中的预后价值。

性别差异是HFpEF研究中另一个需要深入探讨的关键维度。男性和女性对HF及其合并症(包括营养状态)可能表现出不同的病理生理反应。明确营养状态与性别、年龄及合并症等其他危险因素间的交互作用,可能为HFpEF患者提供更具个体化且有效的管理策略。

本研究旨在探讨HFpEF患者中,营养风险评分(NRS-2002)对死亡风险的影响是否会因性别和BMI的不同而产生差异调节。

研究人员对2018年8月至2020年8月期间因急性心力衰竭入院且确诊为HFpEF的患者进行回顾性分析。采用BMI和NRS-2002评估营养状况。NRS-2002可根据近期体重减轻、膳食摄入减少、病情严重程度和年龄评估营养风险。采用逻辑回归模型评估这些营养指标与住院死亡率之间的关联。模型已根据性别、年龄和合并症进行校正。此外,还探讨了NRS-2002、BMI和性别之间的相互作用,以评估营养状况对死亡率的影响是否受这些因素的影响。

结果显示,所分析的HFpEF人群样本显示出基于参与者性别的异质性。虽然营养不良高风险状态不会改变女性患者的死亡风险(P = 0.317),但对男性患者具有显著影响(P = 0.001)。具体而言,BMI为28且NRS-2002评分处于人群典型值(NRS2002=1)的男性患者死亡风险比为0.031,而同等BMI但伴有更高营养不良风险(NRS2002=4)的男性患者,该风险比升至47.51。

在总体样本中,BMI对死亡风险呈负向调节作用(比值比[OR]=0.843,P=0.012)。这一效应在不同NRS-2002评分组间具有一致性,因为NRS-2002评分对BMI与死亡率之间的关系无显著影响(P=0.289)。值得注意的是,这种关联性仅存在于男性患者群体中,女性患者未发现NRS-2002评分与死亡率之间存在类似关联。

综上所述,本研究揭示在HFpEF患者中,NRS-2002评分升高的男性患者院内死亡风险显著增加——评分每升高1个单位,其死亡风险约增加11.565倍,但该关联在女性患者中未观察到。较高BMI通常与较低死亡风险相关,这种保护效应在整个研究人群中保持稳定。本研究结果强调,需开展个体化营养评估和针对性干预,尤其应考虑性别与营养状况对HFpEF患者死亡风险的差异化影响机制。
